The paper version of the CMSARA membership form which can be downloaded by clicking he Adobe Acrobat PDF file icon below. The form is for both new member enrollments, as well as current member renewals. After printing, simply fill o the form and bring it to the next monthly meeting. You may also complete the form and mail it with a check for $20 to PO Box 892, Brandon, MS 39043.
